PRP for Plantar Fasciitis: A Longer-Lasting Answer to Chronic Heel Pain

What Plantar Fasciitis Actually Is

Plantar fasciitis is the most common cause of chronic heel pain. Modern understanding shows that chronic cases are a degenerative fasciopathy. This means tissue breakdown, not a purely inflammatory problem.

Why Cortisone Helps Early Then Fades

Cortisone reduces pain fast by calming inflammation. But the benefit fades. It does not rebuild the degenerated fascia tissue.

Corticosteroid injections to the heel carry real risks. An AOFAS member survey reported plantar fascia rupture in about 1.5 percent of cases. Heel fat pad atrophy occurred in about 1.4 percent. With repeated injections, the rupture rate rises to about 2.4 percent. Because of these risks, guidance is to limit injections to about 2 to 3 per year. Fat pad atrophy reduces the natural cushion under your heel and can make pain worse.

What the Evidence Shows: PRP vs Cortisone

A 2025 systematic review and meta-analysis pooled 24 randomized controlled trials with 1,653 participants to compare PRP and corticosteroid injections for plantar fasciitis.

One important caveat: PRP was not clearly superior to cortisone in the very short term or at one year for pain. PRP is also still considered investigational for this use and is not FDA-cleared as a drug.

Ultrasound guidance helps place PRP accurately into the degenerated plantar fascia. This precision avoids sensitive structures and targets the area of tissue breakdown directly.

Tight calf muscles, foot mechanics, and pelvis alignment all load the plantar fascia. If the upstream chain is not addressed, the heel keeps getting pulled under tension.
